#15 A temperature violation (Naglaa)



Temperature violation  of heat sensitive product in health facility providing immunization service, These photos were taken in February 2013 during my mission to assess the health facilities. The first 2 photos  VVM grade 4 and the expiry 8/2013 of OPV.
The 3rd photo shows the thermometer reading is 12 .
